Please complete the following forms and submit no later than April 21st to the below link:

  1. Camp Staff Members Check List

  2. Confidentiality Agreement

  3. Direct Deposit Form (if you would like your paycheck directly deposited to your account)

  4. Emergency Contact Form

  5. G-4 Form

  6. I-9 Form - Please make sure you submit a copy of two forms of identification that correspond to the I-9 Form (driver's license/photo ID AND signed Social Security Card) or you could just send a copy of a US Passport
    **This form must be opened in Adobe to complete and     must have a real signature. You will have to download the form to your computer and then open in Adobe. You can either upload a scanned copy or a picture of the form. Also, if you send a copy of your Social Security Card, please make sure it is signed.

  7. W-4 Form

  8. Minute 65 (needed for any staff that will bring their own vehicle to camp and transport other staff members)

  9. Employee Manual Acknowledgment (manual will be available at Camp)

  10. Background Check for Minors (must be completed by Parent/Guardian)

  11. Statement for Working with Minors

  12. Voluntary Self-Identification of Race and Gender (2 pages)

  13. Voluntary Self-Identification of Disability (2 pages)

  14. Weapons Policy Acknowledgement
